Job summary

Sentara Albemarle Medical Center in Elizabeth City, NC is hiring an Inpatient Case Manager RN for the Emergency Department. This full-time, 40-hour position offers a $20,000 sign-on bonus for qualified candidates and a daytime shift from 10:00 AM to 7:30 PM EST, Monday–Friday, on-site at 3050 Halstead Boulevard Extended.

You will manage patient transitions, coordinate care plans, and collaborate with the care team to ensure clinical appropriateness and shorter lengths of stay.

Qualifications

• Bachelor's degree in Nursing preferred.
• Registered Nurse (RN) license in NC/VA or Compact/Multi-state required.
• BLS within 90 days of hire.
• Case Management certifications ACM/CCM/CCCTM/CMAC/CMGT-BC required within one year of eligibility.

Responsibilities

• Provide comprehensive care management and facilitate patient transitions to appropriate levels of care.
• Directly support hospitalized patients identified for comprehensive case management.
• Promote appropriate utilization and monitoring of health services; collaborate with care team and patient/caregiver to manage care transitions.
• Develop and implement comprehensive care plans addressing physical, mental and psychosocial needs.
• Operate in hospitals-only settings as part of the care management team.
