Sentara Norfolk General Hospital in Norfolk, VA is seeking an Inpatient Case Manager RN to coordinate care transitions and ensure clinical appropriateness. This full-time role offers a $20,000 sign-on bonus for qualified candidates and a standard 40-hour week with rotating weekends.

The RN must hold an active VA/NC or multi-state license, have a Bachelor's in Nursing, BLS, and pursue ACM/CCM certification within a year. Competitive benefits and compensation are provided.

Qualifications

• Bachelor's Degree in Nursing required or preferred.
• Active RN license in VA, NC or Compact/Multi-state.
• BLS certification within 90 days of hire.
• CCM/ACM certification within one year of eligibility.
• 1 year nursing experience; 1 year acute care case management preferred.

Responsabilités

• Coordinate care across hospital and transitions to appropriate level of care.
• Develop and implement comprehensive care plans based on assessment and evaluation.
• Collaborate with healthcare team to monitor utilization and length of stay.
