Job summary

Sentara Albemarle Medical Center in Elizabeth City, NC, seeks an Inpatient Case Manager/RN for the Emergency Department. This full-time role offers up to a $20,000 sign-on bonus for qualified candidates and operates 10:00 AM to 7:30 PM EST, Monday through Friday.

The RN will lead comprehensive care coordination and facilitate patient transitions to appropriate levels of care, ensuring clinical appropriateness and efficient LOS, while collaborating with the care team and caregivers.

Qualifications

• 1 year of nursing experience required.
• 1 year acute care case management preferred.
• Emergency Dept (ED) or Intensive Care Unit (ICU) experience preferred.

Responsibilities

• Coordinate comprehensive care management and transition planning for patients across the care continuum.
• Support hospital patients identified for case management involvement.
• Collaborate with the care team and patients/caregivers to manage care transitions and utilization of services.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
