MedStar Health is seeking an LCSW-C/LICSW Inpatient Case Manager PRN to join our team at Washington Hospital Center in Washington, DC. The LCSW-C/LICSW serves on the Case Management Team, facilitating delivery of quality, cost-effective, patient-centered care from pre-admission through post-discharge, coordinating individualized plans and ensuring timely discharges with a multidisciplinary team.

Qualifications

• MSW from CSWE-accredited school (Required)
• 1-2 years experience in social work, preferably in a hospital setting (Preferred)
• 1-2 years experience in care/case management (Preferred)
• LCSW-C - Licensed Clinical Social Worker (Required) in DC or MD depending on work location
• LICSW - Licensed Independent Clinical Social Worker (Preferred) in DC or MD depending on work location
• CCM - Certified Case Manager (Preferred)

Responsibilities

• Collaborates with the multidisciplinary team to assess patient needs and coordinate individualized care plans, enabling safe, timely discharges.
• Conducts psychosocial assessments and connects patients with behavioral health resources.
• Facilitates communication among patients, families, and providers to address social, emotional, cultural, and financial barriers to care.
• Documents assessments, referrals, and interventions adhering to policies and regulatory requirements.
• Participates in quality improvement initiatives and professional development to enhance case management effectiveness.

Education

Master’s degree in social work from a CSWE-accredited program
