MedStar Health is seeking an Inpatient Case Manager at MedStar Montgomery Medical Center. The Social Worker will liaison between hospital and community, facilitating smooth transitions from acute hospital care to post-hospital services, within a major health system recognized for its healthy workplace initiatives.

The role emphasizes biopsychosocial assessments, discharge planning, and collaboration with the care team to meet patient needs and improve flow.

Formación

• Experience in hospital social work and discharge planning.
• Biopsychosocial assessment experience.
• Strong teamwork with multidisciplinary teams.

Responsabilidades

• Provide social work and discharge planning services, including assessment, planning, and intervention for patients and families.
• Coordinate with hospital staff and community agencies to facilitate post-discharge care.
• Document assessments, plans, interventions, and outcomes in medical records per policy.
• Support ongoing education and orientation of new staff and supervision of students if applicable.
• Maintain licensure and stay current with continuing education requirements.
• Contribute to quality improvement initiatives within the department.
