MedStar Georgetown University Hospital is seeking an Inpatient Case Manager to coordinate care from pre-admission through post-discharge, promoting quality, cost-effective, patient-centered care within the acute hospital setting.

You will collaborate with multidisciplinary teams to develop and implement comprehensive discharge plans, monitor care across the continuum, and refer to community resources as needed.

Qualifikationen

• Master's degree in social work from a school accredited by the Council on Social Work education.
• 1-2 years' experience in social work, preferably in a hospital setting.
• 1-2 years' experience in care/case management preferred.
• LCSW-C - Licensed Certified Social Worker - Clinical License in the District of Columbia or the State of Maryland depending on work location OR LICSW - Licensed Independent Clinical Social Worker License in the District of Columbia or the State of Maryland depending on the work location.
• CCM - Certified Case Manager preferred.

Aufgaben

• Demonstrate the ability to develop a plan of care across the continuum with long and short-term goals and family involvement.
• Collaborate with and complete referrals to appropriate community agencies for assistance and initiate referrals timely.
• Facilitate communication between patient, family, and healthcare team and develop a comprehensive discharge plan with internal/external providers.
• Complete psychosocial history or socioeconomic assessment in a timely manner per policy standards.
• Develop and maintain information on community resources and referral requirements.

Ausbildung

Master's degree in social work

LCSW-C or LICSW license (DC/MD depending on location)

CCM - Certified Case Manager (preferred)
