Qualifications

• MSW from CSWE-accredited school (Required)
• 1-2 years experience in social work, preferably in a hospital setting (Preferred)
• 1-2 years experience in care/case management (Preferred)
• LCSW-C - Licensed Clinical Social Worker (Required) in DC or MD depending on work location
• LICSW - Licensed Independent Clinical Social Worker (Preferred) in DC or MD depending on work location
• CCM - Certified Case Manager (Preferred)

Responsibilities

• Collaborates with the multidisciplinary team to assess patient needs and coordinate individualized care plans, enabling safe, timely discharges.
• Conducts psychosocial assessments and connects patients with behavioral health resources.
• Facilitates communication among patients, families, and providers to address social, emotional, cultural, and financial barriers to care.
• Documents assessments, referrals, and interventions adhering to policies and regulatory requirements.
• Participates in quality improvement initiatives and professional development to enhance case management effectiveness.

General Summary of Position

MedStar Health is looking for an LCSW-C/LICSW Inpatient Case Manager PRN to join our team at Washington Hospital Center! The LCSW-C/LICSW serves a member of the Case Management Team. Facilitates the delivery of quality, cost effective, patient-centered care from pre-admission through post-discharge timeframe. Ensures the care is designed to meet individualized patient outcomes. Monitors the care and services delivered to selected patient populations during the acute hospital stay working collaboratively with the multidisciplinary team both internal and external to the organization, promotes effective case management and utilization of resources facilitating the continuum of care, and works to achieve optimal clinical and resource outcomes for the acute, and posthospital phases of care.

Primary Duties:

Qualifications:

• Master’s degree in social workfrom a school accredited by the Council on Social Workeducation (Required)
• 1-2yearsExperiencein social work,preferablein a hospital setting(Preferred)
• 1-2yearsExperiencein care/case(Preferred)
• LCSW-C - Licensed Certified Social Worker -Clinical Licensein the District of Columbia or the State of Maryland depending on work location (Required)

This position has a hiring range of

USD $74,214.00 - USD $134,596.00 /Yr.
