Job summary

MedStar Montgomery Medical Center in Maryland is seeking an Inpatient Case Manager to join our social work team. The Social Worker serves as a patient liaison between hospital and community, facilitating a smooth transition from acute care to post-hospital services.

You will conduct biopsychosocial assessments, develop discharge plans, counsel patients and families, and collaborate with health care teams to ensure timely and effective patient flow.

Qualifications

• Master's degree in social work (MSW) required.
• LMSW/LGSW licensure in Maryland preferred.
• 1-2 years hospital social work experience preferred.

Responsibilities

• Provide social work and discharge planning services, including assessment, planning, and intervention.
• Identify patients needing social work intervention through case finding mechanisms.
• Develop discharge plans responsive to patient and family concerns and coordinate with care teams.
• Maintain documentation and collaborate with community providers for post-discharge needs.
