Job summary

Select Specialty Hospital in Plano, TX is seeking a Case Manager to oversee discharge planning, utilization reviews and social work services for chronically ill patients in a LTACH setting. This full-time role requires collaboration with physicians, nurses, respiratory therapists and rehab staff to ensure timely, safe patient transitions.

You will report to the Director of Case Management, document thoroughly, optimize insurance reimbursement, and maintain compliance with governing guidelines

Qualifications

• Current licensure in a clinical discipline (RN/LPN/LVN) or RT, or Social Work license per state guidelines.
• Inpatient hospital experience in critical care/acute care settings preferred.

Responsibilities

• Develops and implements a patient-specific discharge plan.
• Performs verification of utilization criteria reviews.
• Coordinates with payors to maximize reimbursement for services.
• Maintains compliance with Utilization Management policies and procedures.
• Facilitates multi-disciplinary team meetings with physicians, nurses, therapists.
