Job summary

Qualifications

• 1–2 years’ experience.
• Maryland licensure as Social Worker is required.
• Master's degree in social work preferred.

Responsibilities

• Provides social work and discharge planning services including assessment, planning, and intervention.
• Coordinates activities with hospital staff and referral agencies to facilitate patient care and discharge planning needs.
• Documents assessments, plans, interventions, and outcomes in the medical record and reports as required.
• Supports and mentors colleagues, participates in quality improvement projects, and contributes to staff training.
• Maintains knowledge, certification, and licensure; participates in continuing education and hospital training as required.

Education

Bachelor's degree in Social Work

Maryland licensure as Social Worker

Master's degree in Social Work preferred
