Baltimore Washington Medical Center is seeking a Care Manager to coordinate patient care across the continuum. The role requires collaboration with physicians, nursing staff, and families to develop safe, individualized plans and ensure timely decision-making.

You will lead discharge planning efforts, connect patients with community resources, and assist with advanced directives and housing referrals while engaging in interdisciplinary rounds to optimize patient outcomes.

Formación

• Graduation from an accredited School of Nursing with current Maryland nursing licensure.
• BSN or MSN preferred.
• 3-5 years of relevant hospital case management experience.
• Ability to collaborate with multi-disciplinary teams.

Responsabilidades

• Collaborates with healthcare team, community resources, and patient/family to plan and manage care.
• Provides care management activities to ensure an effective, individualized and safe plan of care.
• Supports discharge planning and connects patients to community resources and crisis services.
• Assists with advanced directives, guardianship, homelessness issues, and referrals as needed.
