PAM Health in Colorado is seeking a Case Manager to coordinate interdisciplinary care for designated patient populations. The role focuses on reviews of inpatient records to ensure appropriate resource use, discharge planning, and collaboration with the care team to meet financial and quality goals.

Ideal candidates have state licensure, 3–5 years inpatient experience (acute/IRF/LTACH preferred), and a current BLS certification.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Discipline options include occupational therapist, physical therapist, RN, respiratory therapist, SLP, or social worker.
• Current BLS certification required.

Responsibilities

• The Case Manager is responsible for the interdisciplinary coordination of care for a designated patient population.
• The Case Manager will also maintain and enhance payor relationships.
