You will assess discharge needs, collaborate with multiple disciplines, and work to strengthen payor relationships while supporting optimal patient outcomes.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include OT, PT, RN, RT, SLP, or social worker.
• Current BLS certification required.

Responsibilities

• Case Manager coordinates interdisciplinary care for a designated patient population.
• Reviews inpatient records to ensure proper resource utilization and admission level.
• Assesses discharge needs and coordinates the discharge plan with the team.
• Maintains and enhances payor relationships.
