PAM Health Specialty Hospital of Pittsburgh is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population. You will review inpatient admission records to ensure appropriate resource use and determine admission level, assess discharge needs, and arrange a suitable discharge plan.

You will collaborate with the interdisciplinary team to achieve quality outcomes and maintain payor relationships, with licensure in the relevant state and current BLS required.

Formación

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
