Qualifications

• Licensure in the state where the hospital resides is preferred; current BLS certification required.

Responsibilities

• Coordinate care for designated inpatient population with the interdisciplinary team.
• Review inpatient admission records to ensure proper utilization and determine admission level of care.
• Assess discharge needs and coordinate appropriate discharge plan.
• Maintain and enhance payor relationships to support financial and quality outcomes.
