Advocate Health is seeking a Nurse Case Manager in Illinois to lead assessments, care planning, and discharge coordination across patient care teams. You will collaborate with Social Work and medical staff to ensure timely transitions, patient satisfaction, and regulatory compliance in a fast-paced hospital setting.

The role requires RN licensure with 3 years of related experience; BSN is preferred. A comprehensive benefits package and a supportive environment are provided.

Competenze

• Graduate of a nursing program with RN licensure.
• 3 years of related nursing experience preferred.
• Knowledge of discharge planning and care coordination.
• Experience with utilization management and case management preferred.

Mansioni

• Conducts assessments, plans, and initiates interventions within timeframes.
• Provides continuity of care and discharge planning per standards.
• Manages case coordination across health care, finances, housing and family needs.
• Initiates referrals and documents discharge planning and reviews.
