Qualifikationen

• Certification/Registration: RN or SW license preferred within state; ACM/CCM within 1 year or equivalent.
• Experience: 3 years of experience in a relevant clinical setting.

• Provides management oversight of daily operations of care management and social work programs at the site.
• Monitors staff schedules, timecards, and PTO to ensure continuity of services.
• Develops and leads performance improvement activities and measures.
• Collaborates with hospital leaders, physicians, and units to optimize discharge planning and care coordination.
• Ensures compliance with CMS, OSHA, accrediting bodies, and state/federal guidelines.
