Job summary

Carolinas Medical Center seeks a Manager to lead its inpatient Care Management and Social Work programs and to directly manage and develop site staff. This role collaborates with hospital leaders, physicians, nurses, and post-acute partners to support timely care progression and discharge planning while ensuring regulatory compliance.

Leadership and inpatient case management experience are strongly preferred, with opportunities to drive performance improvement, team development, and budget

Qualifications

• Bachelor's Degree in Nursing or Master's Degree in Social Work or related field.
• 3 years of experience in a relevant clinical setting.
• Certifications within 1 year: ACM, CCM, RN-BC, or C-SWHC (as applicable).

Responsibilities

• Oversee daily operations of care management and social work programs at the site(s).
• Monitor staff schedules, PTO, and timecards to ensure continuity of services.
• Lead performance improvement activities and set targets to improve care processes.
• Collaborate with physicians, leadership, and post-acute partners to ensure compliant discharge planning.
