Job summary

Qualifications

• Registered Nurse license in the state of practice.
• Bachelor’s Degree in Nursing or related field.
• Typically requires 5 years of experience in clinical nursing or 1-2 years of care management.

Responsibilities

• Facilitates communications among patient/family and care teams to resolve barriers to timely care.
• Collaborates with patient/family and inter-professional teams to ensure quality, efficient care.
• Uses evidence-based approaches to increase patient activation and engagement; helps develop SMART goals.
• Identifies barriers to learning and to delivery of care; reports findings and develops plans.
• Manages CM caseload per department expectations; completes tasks and documentation for MCO and regulatory requirements.
• Coordinates patient information and transitions between facilities and care teams.
• Coordinates referrals to internal departments and external resources as needed.
• Demonstrates knowledge to provide age-appropriate care; adheres to department policies.
