Job summary

Carolinas Medical Center seeks a Manager to lead its inpatient Care Management (CM)/Social Work (SW) program and directly manage and develop site staff.

This leader will collaborate frequently with hospital leaders, post-acute partners, physicians, physician advisors, nurses, and others to support accurate, timely care progression and discharge planning while ensuring compliance with regulatory standards and conditions of participation related to discharge planning and utilization management.

Qualifications

• Bachelor's Degree in Nursing or Master's Degree in Social Work or related field.
• Leadership and inpatient case management experience strongly preferred.
• Knowledge of discharge planning, utilization management, and regulatory standards.

Responsibilities

• Oversee daily operations of care management and social work programs at the site.
• Approve staff schedules, PTO, and timecards to ensure service continuity.
• Monitor staff proficiency and develop improvement plans to meet regulatory standards.
• Lead performance improvement activities and establish targets and benchmarks.
• Analyze data to identify care process variances and improvement opportunities.
• Collaborate with physicians, nurses, and leaders on discharge planning and care progression.
