Qualifications

• Minimum of two years combined RN experience across Med/Surg, ICU, ED, Home Health, SNFs, Hospice, LTAC, Inpatient Rehab, Utilization Management.
• Successful completion of an RN program by date of hire; HS Diploma or GED required.

Responsibilities

• Coordinate post-discharge care to ensure timely and effective hospital discharge for routine and complex patients.
• Complete and document patient assessments that are timely, age appropriate, and consider psychosocial support and health plan benefits.
• Collaborate with patients, families, and care teams to develop plans of care maximizing resource use.
• Facilitate safe transfers and transitions to appropriate care levels while maintaining quality and utilization standards.
• Maintain ongoing contact with interdisciplinary staff and care services for safe transitions across settings.
• Ensure regulatory compliance in collaboration with care teams.
• Perform duties as requested.
