Qualifications

Responsibilities

• Uses age appropriate assessment and intervention skills.
• Assists members in developing cost effective, appropriate health care plans through optimizing resources.
• Promotes multi-disciplinary, patient-centered care considering personal, psychological, economic, and cultural needs.
• Maintains knowledge of Kaiser Permanente services/benefits, community resources, and relevant laws.
• Completes required documentation of services and social work plan of care.
• Participates in program development, orientation, and education to empower members.
