Job summary

College Medical Center is seeking a Behavioral Health Social Worker to collaborate with the Case Management Team, physicians, and families in assessing and planning care for patients with complex biomedical and psychosocial needs.

The role supports inpatient nursing staff with discharge planning, ensures quality oversight of non-licensed social work staff, and participates in team meetings to resolve needs. Monday–Friday day shift with compensation variations based on experience.

Qualifications

• MSW or MFT/LMFT with active license is required.
• Current BLS for healthcare providers (American Heart Association) required.
• CPI certificate within 30 days of hire required.
• Five years preferred in social services support, behavioral health setting advantageous.

Responsibilities

• Lead interviews and assessments for individuals with complex behavioral health needs.
• Develop integrated behavioral health treatment goals within a comprehensive plan.
• May conduct group or individual therapy as program needs require.
• Provide timely documentation on complex biomedical and psychosocial issues.
• Manage Behavioral Health referrals and guide patients and physicians to access services.
• Collaborate in discharge planning for safe transition home or to another level of care.
• Coordinate with families, Case Management Team and providers on the care plan.
• Promote access to Federal, State, and County assistance programs.
