Job summary

AlohaCare is seeking a Behavioral Health Inpatient Engager in Hawaii to perform face-to-face engagements with inpatient or recently discharged members. You will coordinate with hospital staff, Health Coordination, Care Navigators, and community providers to support transitions to outpatient care.

Responsibilities include biopsychosocial assessments, individualized transition plans, and collaboration with families. Remaining compliant with HIPAA and state/federal regulations is essential.

Responsibilities

• Conduct in-person outreach with inpatient and recently discharged members.
• Perform biopsychosocial assessments and identify determinants affecting recovery.
• Develop individualized transition and service plans.
• Coordinate with hospital staff and community providers to ensure seamless care.
• Collaborate with family members and caregivers as appropriate.
• Conduct follow-up contacts to monitor stabilization and engagement.
• Facilitate transitions from hospital to outpatient care.
• Coordinate follow-up appointments and linkages to services.
• Address housing, transportation, medication access, and financial barriers.
