Transformation Health Network in Hawaii seeks a licensed behavioral health clinician to manage an outpatient caseload, conduct intake assessments, and co-create individualized treatment plans. You will provide individual, family, and group therapy, facilitate psychoeducation sessions, and document encounters in the EHR.

Collaboration with psychiatry, primary care, and social services is essential. Two years of relevant outpatient experience are preferred.

Formación

• Master's degree in psychology, social work, marriage and family therapy, counseling, or related field.
• Licensed or licensed-eligible in Hawaii (LMFT, LCSW, LMHC or equivalent) and able to obtain clearance.
• Background check, CPR/First Aid, TB clearance may be required after hire.
• Minimum two years of counseling or behavioral health experience, outpatient preferred.

Responsabilidades

• Manage a client caseload within an outpatient behavioral health clinic.
• Conduct intake assessments including biopsychosocial evaluations.
• Develop, implement, and update individualized treatment plans and goals.
• Provide individual, family, and group counseling using evidence-based practices.
• Facilitate therapeutic groups and psychoeducation sessions.
• Document clinical encounters and case notes in EHR promptly.
• Provide crisis intervention and outpatient stabilization support.
• Coordinate care and referrals to psychiatry, primary care, and social services.
• Ensure warm handoffs and continuity of care across services.
• Collaborate with schools, courts, and external partners with consent or orders.
• Attend team meetings, case conferences and clinical supervision.
• Engage in ongoing training and professional development.
