Qualifications

• High school diploma or equivalent.
• HCPS certification required.
• Strong written and oral communication skills.
• Proven customer-service orientation and problem-solving ability.

Responsibilities

• Perform initial outreach education and welcome calls as needed.
• Schedule and attend member home visits and provider visits.
• Coordinate with RN/SW Health Coordinators to complete reassessments.
• Document in an electronic health record and coordinate referrals.
