Job summary

Kaiser Permanente Hawaii is seeking a registered nurse to provide case management and outreach for Health Plan members hospitalized at community hospitals, applying risk, quality and utilization management concepts in daily activities.

The role involves coordinating resources, discharge planning, and collaborating with Continuing Care providers and PCP teams to ensure cost-effective, high-quality care. Hawaii nursing license and BLS are required.

Qualifications

• Minimum one year working with low income/high risk patients.
• Bachelor's degree in nursing OR four years of related field experience.
• Hawaii RN license required at hire.
• Basic Life Support from American Heart Association

Responsibilities

• Implements Continuing Care programs with providers and PCP Team; adapts to site needs and monitors outcomes.
• Provides case management and coordinates resources to assure continuity of care.
• Monitors high-risk members and assists them with access to care.
• Assists patients/families with identified problems/concerns.
• Meets with Continuing Care providers and PCP Team to monitor targeted patients and coordinate community care.
• Makes daily hospital rounds for early identification of UM and discharge planning activities.
• Reports any Risk/Quality/Utilization Management issues to Neighbor Island Management Team.
• Acts as contact person for MOA/Oahu discharges and assists patients with smooth transfer to home.
• Participates in community hospital interdisciplinary team meetings.
• Maintains confidentiality of all patient information.
