Job summary

AlohaCare in Hawaii seeks a Care Navigator to help QUEST members understand benefits and access providers and services. You will reach out by phone to ensure members obtain in-network care, community resources, and plan benefits under the Interdisciplinary Care Team’s direction.

Primary duties include completing welcome calls, conducting health risk assessments, and coordinating referrals. This role requires 2+ years in a customer service or call center setting and strong communication skills.

Qualifications

• Minimum 2 years in customer service or call center roles.
• Ability to communicate with diverse populations.
• Proficient with Microsoft Word/Excel/Outlook/PowerPoint.
• Experience with EMR or care management systems.

Responsibilities

• Perform welcome calls for newly enrolled QUEST members.
• Conduct telephonic health risk assessments as needed.
• Provide telephonic support for member and provider calls.
• Coordinate referrals and prior authorizations per member needs.
• Document assessments in an electronic care management system.
• Collaborate with interdisciplinary care team members.
