ChenMed is seeking a Primary Care Physician to join our mission-driven team serving seniors and delivering excellent, affordable VIP care. You will provide comprehensive care, manage a panel of up to 450 patients, and lead a care team to improve health outcomes in a value-based system.

You will work closely with hospitalists and other health care professionals, adhere to guidelines, participate in risk and quality programs, and pursue partnership through accountability, compassionate leadership,

Qualifications

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
Must be able to obtain a State Medical License for the state(s) in which he/she will be working.
Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
Board Eligibility is required.
Once Board certified, PCP will maintain board certification by doing necessary MOC, CME and/or retaking board exams as required.
Must have a current DEA number for schedule II-V controlled substances.
BLS certification from the AMA or American Red Cross required within first 90 days of employment.

Responsibilities

The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed.
PCP will engage with the hospitalist whenever one of their patients is in the hospital.
PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation.
PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator.
For patients unable to come to the office, PCP will engage with the transitional care team and others depending on market.
PCP will take an active role in recruiting patients for the center and additional providers for the company.
Supervises, collaborates with, or functions with an APP and remains accountable for the actions of the APP.
Performs other duties as assigned and modified at manager’s discretion.
The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
