Job summary

The Home Care Infusion RN at Hackensack Meridian Health provides professional nursing care to patients in the home, assessing, planning, implementing, and evaluating care according to policies and infusion therapy standards.

Responsibilities include following physician orders, documenting care in the EMR, coordinating with the care team, and teaching patients to meet plan-of-care goals. Requires NJ RN license and IV/infusion experience.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of one (1) year of RN experience.
• Ability/Experience with inserting IVs.
• Proficiency with Google Suite and Microsoft Office (preferred).

Responsibilities

• Provide professional nursing care to patients in their home.
• Demonstrate infusion therapy competency.
• Ensure timely adherence to physician orders and documentation.
• Collaborate with interdisciplinary team and coordinate care.
• Maintain Plan of Care and update EMR with changes.
• Deliver teaching to patients and families to meet care goals.
