Job summary

Hackensack Meridian Health At Home is seeking a Home Care Infusion RN to deliver professional nursing care in patients' homes, focusing on infusion therapy. You will assess, plan, implement, and evaluate care while coordinating with the care team and ensuring documentation meets standards.

The role requires an Associate's Degree in Nursing or a Diploma in Nursing, 1+ year RN experience, IV insertion capability, and excellent communication.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• 1+ year of RN experience.
• Experience inserting IVs.
• Excellent written and verbal communication skills.
• Proficient computer skills (Google Suite and MS Office).

Responsibilities

• Provide professional nursing care at patients' homes, including infusion therapy.
• Ensure nursing care follows physician orders and documents progress.
• Document care and plan in the EMR, collaborating with the care team.
• Identify payor requirements and maintain authorization ranges.
• Update care plans and conduct patient/family teaching to meet goals.
• Ensure safety, adhere to policy, and maintain accurate on-call communications.
