Job summary

The Home Care Infusion RN at Hackensack Meridian Health At Home provides professional nursing care to patients in their homes, performing assessment, planning and infusion therapies in accordance with agency policies. This role emphasizes delivering high-quality, patient-centered care in a flexible home setting.

Qualifications

• RN with an Associate's degree or nursing diploma.
• Minimum of one year of RN experience.
• IV insertion experience preferred.
• NJ State RN license and BLS required.
• Infusion competency testing and ongoing education.

Responsibilities

• Provide professional nursing care to patients in the home setting per plan of care.
• Ensure orders are followed and documentation meets policy and regulatory standards.
• Document care in the EMR and coordinate with the care team.
• Identify payor authorizations and assist with required documentation.
• Deliver holistic, safe care and patient/family teaching as assigned.
