Job summary

Hackensack Meridian Health At Home is seeking a Home Care Infusion RN to deliver professional nursing care in patients' homes, assessing and addressing infusion needs with 1:1 attention. You will implement plans, document care, and coordinate with physicians within an interdisciplinary team.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of one year of RN experience.
• Ability/Experience with inserting IVs.
• With previous and recent IV experience (preferred) within one (1) year: Demonstration with preceptor inserting peripheral line, accessing ports, PICCs and central lines.
• Without previous experience or experience not within one (1) year: Will complete a vascular access device (VAD) course through HMH and follow up practicum experience in HMH outpatient infusion setting and return demonstration with preceptor inserting peripheral line, accessing ports, PICCs and central lines
• Successful Completion of Infusion Test within probationary period with grade of 80% or higher
• Excellent written and verbal communication skills
• Proficient computer skills that include Google Suite and/or Microsoft Office platforms

Responsibilities

• The Home Care Infusion RN provides professional nursing care to patients in their home, assessing, planning, implementing and evaluating home nursing needs.
• Assure compliance with physician orders and that visit frequency follows orders; document progress toward discharge goals.
• Document care in EMR, collaborate with an interdisciplinary team and ensure safe, holistic patient care.
• Identify payors and maintain authorizations within required guidelines.
• Update care plan within two weeks of status change and communicate effectively with care team.
• Provide patient teaching to meet goals and document teaching in the medical chart.
• Coordinate with physicians and manage referrals for appropriate resources.
• Maintain productivity, adhere to standards, and demonstrate flexibility in duties.
• Lift at least 50 lbs., push/pull 50 lbs., and stand for extended periods.
