Job summary

Hackensack Meridian Health At Home is seeking an experienced Home Care Infusion RN to deliver professional nursing care in patients' homes, performing assessments, planning, and evaluating care plans with a focus on infusion therapy. You will work with an interdisciplinary team to ensure safe, high-quality care and documentation in the EMR.

The role requires an RN license in New Jersey, 1+ year of RN experience, and IV insertion skills.

Qualifications

Associate's Degree in Nursing or Diploma Certificate in Nursing.
Minimum of one (1) year RN experience.
Ability/Experience with Inserting IVs.
Bachelor of Science in Nursing Degree (BSN) preferred.
NJ State Professional Registered Nurse License.
AHA Basic Health Care Life Support (BLS) Certification.

Responsibilities

Provide professional nursing care to patients in home settings with assessment, development, implementation and evaluation of care plans.
Ensure compliance with physician orders and timely documentation.
Document care activities in the EMR system and communicate with the interdisciplinary team.
Coordinate with physicians, managers and other health care team members for treatment and referrals.
On-call availability and after-hours responsiveness as required.
Maintain adherence to organizational policies and standards of practice.
