Job summary

Option Care Health, Inc. seeks an Infusion Nurse II to coordinate and provide intermediate direct patient care for home- and alternate infusion-site patients under physician plan of care. Works under structured supervision to ensure patient safety and continuity of care.

Position requires an active RN license, CPR certification, and at least 1 year of nursing experience. May involve travel up to 100% and requires adherence to HIPAA and company policies.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families.
• Modifies plan of treatment in response to changing patient status or physician orders.
• Assesses patient needs and physical status at each skilled visit and documents care provided.
• Provides training and mentorship to Infusion Nurse I’s.
• Communicates with clinical teams, patients, and families to ensure continuity of care.
• Prepares discharge summaries and reports patient progress on transfer.
• Follows HIPAA and other regulatory guidelines, and maintains charting and billing documentation.
