Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an alternate infusion suite. You will work under supervision to ensure patient safety and continuity per a physician's plan of care.

The role requires an active RN license, 1 year of nursing experience, and CPR certification. Strong communication and documentation skills are essential for billing and care coordination in a fast-paced infusion setting.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required (in-person class per company policy).

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit, re-evaluates needs and informs physician of changes.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Provides patient and family teaching to achieve independence with prescribed therapy and care goals.
• Communicates with Clinical team, medical providers, patients, and families to facilitate continuity of care; documents in real-time.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
• Demonstrates compliance with policies, regulatory guidelines, and accreditation standards.
• Maximizes work efficiency using computers to validate orders and document care.
• Responds to patient requests and initiates emergency procedures as needed.
• Follows HIPAA and safeguarding confidentiality; participates in team conferences and precepts new staff.
• Inserts PICC/Midlines per physician order; troubleshoots/declots central lines; educates on line care.
