Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an alternate infusion suite, under a physician’s plan of care. You will work under the supervision of a designated supervisor to ensure patient safety and continuity of care.

The role requires an active RN license in the practice state, a minimum of 1 year nursing experience, and CPR certification.

Qualifications

• RN license active in state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and manages intermediate nursing plan of care treatments in home or alternate infusion suite.
• Assesses patient needs and status at each visit, updates physician orders as needed.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Educates patient and family to achieve independence with prescribed therapy and care.
• Communicates with clinical team, patients, and families to ensure continuity of care and timely documentation.
• Coordinates discharge planning and prepares discharge summaries with clear patient instructions.
