Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or alternate infusion suite under a physician’s plan of care. Works under structured supervision of the designated supervisor.

The role emphasizes patient safety, documentation, and collaboration with the clinical team to ensure continuity of care across settings.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 years of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments.
• Assesses patient needs and physical status at each skilled visit.
• Provides training and mentorship to Infusion Nurse I's as needed.
• Communicates with Clinical team, patients, and families to facilitate continuity of care.
• Documents care in real-time and submits per policy.
• Observes compliance with policies and regulations.
