Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate home-based patient care under a physician’s plan. You’ll work under supervision to ensure safety and continuity of care.

We value compassionate nurses who educate patients and families, document care in real-time, and collaborate with clinical teams to optimize outcomes across home infusion settings.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs at each skilled visit and informs physician of changes.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Educates patients and families to achieve independence with prescribed therapy.
• Documents care in real-time and submits records for billing compliance.
• Coordinates discharge planning and prepares summaries with patient instructions.
• Ensures compliance with policies, procedures, and regulatory standards.
• Maximizes work efficiency using computers to validate orders and communicate care.
• Responds promptly to patient requests and initiates emergencies when needed.
• Follows HIPAA and safeguarding guidelines for patient information.
• Performs other related duties as directed by supervisor.
• In applicable cases, may insert PICC/Midlines per physician orders.
