Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinat​e and provide intermediate direct patient care in home or alternate infusion suites. The role requires functioning under supervision and ensuring patient safety per physician plan of care.

Responsibilities include initiating nursing plans, educating patients and families, documenting in real time, and coordinating with clinicians to optimize care. Travel up to 100% may be required as part of home infusion services.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Current CPR certification required.
• Basic knowledge of computer systems and software for documentation and scheduling.
• Infusion nursing experience and patient care skills.

Responsibilities

• Initiates and manages intermediate nursing plan of care and treatments.
• Educates patients and families to achieve independence with prescribed therapy.
• Documents care and coordinates with clinical team in real time.
• Assists in discharge planning and transfer of care when needed.
