Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an alternate infusion suite, ensuring patient safety under a physician’s plan of care.

Under structured supervision, the RN will develop and modify care plans, educate patients and families, and document care in real time while collaborating with the clinical team to optimize continuity of care. Travel up to 100% may be required.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Current CPR certification.
• 1–2 years infusion nursing experience preferred.

Responsibilities

• Initiates and implements intermediate nursing plan of care treatments.
• Educates patients and families/caregivers on prescribed therapy.
• Documents care in real-time and communicates with clinical team.
• Coordinates discharge planning and provides hand-off notes.
• Travels to patient homes or infusion suites as needed.
