Job summary

Option Care Health, Inc. is a leading home infusion services provider seeking an Infusion Nurse II to coordinate and deliver intermediate direct patient care in home or infusion suites. You will operate under supervision and ensure patient safety and continuity of care under a physician’s plan.

The role requires active RN licensure, at least 1 year of nursing experience, and current CPR certification. Travel to patients’ homes is expected as part of this position.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.
• Bachelor of Science in Nursing preferred.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit; re-evaluates needs and informs physician of changes.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety.
• Provides effective, patient-centered teaching to achieve independence with prescribed therapy.
• Communicates with Clinical team, providers, patients and families to facilitate continuity of care.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
