Job summary

Option Care Health in the United States is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite under a physician's plan of care.

The role requires an active RN license, CPR certification, and at least 1 year of nursing experience, with opportunities to mentor others and participate in multidisciplinary teams while adhering to HIPAA and policy guidelines.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit.
• Provides training and mentorship to Infusion Nurse I’s.
• Provides effective and safe teaching to patients and families for independence with prescribed therapy.
• Communicates with Clinical team, medical providers, patients, and families to facilitate continuity of care.
• Prepares clinical documentation in real-time during visits and submits to the nursing department.
