Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite, ensuring patient safety under a physician's plan of care. Licensed RN with at least 1 year of nursing experience and CPR certification preferred.

The role involves developing and implementing plans, educating patients and families, collaborating with the clinical team, and maintaining compliant documentation.

Qualifications

• Active RN license in the state of practice and current CPR certification.
• Minimum of 1 year of nursing experience, preferably infusion.
• Must have reliable vehicle and driver's license for home visits.
• Strong clinical documentation and patient education skills.

Responsibilities

• Initiates and implements intermediate nursing plans of care in patients' homes or infusion suites.
• Assesses patient needs, administers medications, and educates patients and families.
• Provides training and mentorship to less-experienced nurses when needed.
• Communicates with the clinical team and documents care in real-time.
• Coordinates discharge planning and follow-up care as indicated.
