Job summary

Soleo Health is seeking a Full-time Infusion Nurse to support our infusion patients in the Roswell, GA area. You will provide direct patient care, assessments, care planning, and education for home infusion therapy in line with our care matrices and best practices.

Responsibilities include coordinating with the care team, educating patients, ensuring safety, and documenting activities. Schedule is 40 hours/week, with on-call requirements and local travel as needed.

Qualifications

• Knowledge of IV access devices including lines and ports.
• Current RN license in the state of practice.
• BSN preferred; advanced infusion certifications optional.
• On-call rotation availability for evenings/nights/weekends.

Responsibilities

• Provide direct care for assigned patients and manage home infusion therapy.
• Coordinate with pharmacists, prescribers, dietitians, and care partners.
• Educate patient and care partner on infusion procedures and safety.
• Assess home environment for safety and appropriateness of care.
• Develop and evaluate plan of care with the care team.
• Document all encounters and ensure regulatory compliance.
