Job summary

Soleo Health is seeking a Full-time Infusion Nurse to support infusion patients in the Duluth, GA area and surrounding communities. The nurse will provide direct patient care, education, and care coordination in home or alternate sites, following care matrices and best practices.

Responsibilities include coordinating with pharmacists, prescribers, and care partners; educating patients and care partners; evaluating home environments; maintaining accurate documentation and participating in nurse

Qualifications

• Knowledge of IV access devices (lines, ports)
• RN license in good standing (state)
• BSN preferred
• On-call rotation, nights and weekends included
• Administrative oversight of workflows and resources

Responsibilities

• Provide direct care for assigned patients and initiate/maintain communication with care team.
• Educate patients/care partners to safely administer home infusion therapy.
• Assess home environment for safety and appropriateness of care.
• Develop and evaluate plan of care with team members.
• Coordinate patient care via approved platform and document encounters as required.
