Job summary

Soleo Health is seeking a Full-time Infusion Nurse to support our infusion patients in Sandy Springs, GA and the surrounding area. The Infusion Nurse provides primary patient care for assigned patients, including assessments, care planning, evaluation and education in home or alternate setting.

Responsibilities include direct patient care, education, documentation, and collaboration with the care team, with on-call rotation and the potential for mileage reimbursement under a competitive

Qualifications

• IV access devices including lines and ports
• Current RN license in good standing
• BSN preferred from an accredited school
• Advanced certifications (CRNI, ONS, IgRN) preferred but not required
• On-call rotation with nights and weekends
• Administrative oversight of workflows and resources
• Emergency availability during non-standard hours
• Minimum 3 years nursing experience; infusion therapy preferred
• ICU and/or Cardiac care experience preferred
• Proficient with Outlook and MS Office
• Valid driver''s license with few restrictions

Responsibilities

• Provide direct patient care including initiation and maintenance of communications with care team members.
• Educate patient and care partner on home infusion therapy and provide instruction.
• Evaluate the home environment for safety and appropriateness of care.
• Develop and evaluate the plan of care with the team.
• Manage scheduling via company platform and communicate changes.
• Coordinate patient care via approved platform.
• Participate in nurse on-call rotation for continuous support.
• Document all patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies and clinical standards.
• Ensure timely POTs and required clinical documents are submitted.
• Ensure patient documents are completed and returned to the branch office within timeframe.
