Job summary

Option Care Health, the nation’s largest home infusion provider, seeks an experienced Home Infusion Nurse II to coordinate and deliver intermediate direct patient care in the home or alternate infusion setting. You will follow a physician’s plan of care, ensure patient safety, and maintain timely clinical documentation.

Responsibilities include initiating care plans, educating patients and families, assessing status at each visit, and communicating with the clinical team.

Qualifications

• Active and unrestricted RN license in the state of practice.
• 1 year of nursing experience (minimum).
• Current CPR certification required.
• Valid driver’s license and access to a vehicle for travel.

Responsibilities

• Initiates and implements intermediate nursing care plans in home or infusion suite.
• Administers medications and educates patients and families.
• Assesses patient needs at each home visit and documents care.
• Coordinates with clinical team and physicians for care plans.
• Provides training to Infusion Nurse I’s to ensure safety and compliance.
