Job summary

Option Care Health, Inc. seeks an Infusion Nurse II to coordinate and deliver intermediate nursing care in patients' homes or infusion suites. You will implement plans of care under physician orders, educate patients and families, and maintain timely documentation for billing and continuity of care.

You will mentor a team of nurses, participate in multidisciplinary rounds, and adhere to HIPAA and policy requirements while traveling up to 100% for patient visits.

Qualifications

• Active and unrestricted RN license in state of practice.
• Current CPR certification required.
• 1+ year of nursing experience preferred in infusion/home care.
• Valid driver's license and vehicle for travel.

Responsibilities

• Coordinate and provide intermediate direct patient care at home or infusion suite.
• Initiate and modify nursing plan of care with physician orders.
• Educate patients and families on therapy and safety.
• Document care in real-time and ensure billing-ready records.
• Mentor Infusion Nurse I's and supervise as needed.
• Perform discharge planning and care coordination with team.
