Soleo Health is seeking a Full-time Infusion Care Coordinator (Patient Ambassador) in Centennial, CO. This role focuses on data entry, scheduling coordination, and patient communication to support our infusion services. 100% onsite with no remote work.

Qualifications

• High school diploma or equivalent required.
• Licensed Practical Nurse/Licensed Vocational Nurse License preferred.
• Prior healthcare scheduling experience required.
• Home infusion experience required.
• Ability to work on-site and interact directly with patients in a clinical setting.
• Strong communication skills.
• Ability to prioritize and multitask.
• Basic computer skills including Microsoft Excel, Word, Outlook.
• CPR Plus application experience preferred.

Responsibilities

• Data entry of clinical information into management system for billing and scheduling.
• Coordinate scheduling activities for the nursing department to ensure accuracy and efficiency.
• Receive referrals, collect insurance information, and verify benefits or prior authorizations.
• Place outbound calls to patients or providers to update care status and next steps.
• Serve as main contact for referral activity with internal/external partners and ensure timely patient communication.
• Provide exceptional customer service to internal and external customers.
